Advance Care Directives – Principles and Practice

  • What is Advance Care Planning
  • Tasmanian ACD document
  • Substituted decision making in Tasmania
  • The place of ‘capacity’ in ACP conversations
  • Ethical and legal questions
  • Role of the doctor and clinical decision making
  • Implementing into practice

Difficult conversations

  • Introducing advance care planning
  • Responding to difficult questions
  • Recognising emotions
  • Providing non-judgemental and supportive care

After these sessions attendees will:
1. Have a better understanding of the principles of advance care planning
2. Be familiar with the Tasmanian Advance Care Directive
3. Understand substituted decision making options in Tasmania
4. Be able to discuss end of life wishes and advance care planning with patients and/or families
5. Feel more confident responding in difficult conversations
6. Have a greater understanding how Advanced Care Planning and End of Life conversations can be integrated into practice


The workshops will include interactive group discussions. There will be practical learning based on case scenarios in having discussions of concerns and wishes for end of life care. Participants will be invited to consider how advance care planning discussions can be incorporated into practice systems.
